مقدمه: رواسکولاریزاسیون از طریق جراحی جهت بیماری آترواسکلروتیک قلبی یکی از بزرگترین دستاوردهای موفق علم پزشکی است. عمل جراحی پیوند بای پاس شریان کرونری (CABG) تا به حال بطور مرسوم به صورت on-pump یعنی با استفاده از بای پاس قلبی - ریوی (CPB) انجام می شده است. عمل جراحی CABG به روش off-pump یک استراتژی جراحی پذیرفته شده است که استفاده از آن رو به افزایش است.
به منظور ارزیابی تاثیر رواسکولاریزاسیون میوکارد به روش off-pump، مطالعات گوناگونی در سطح دنیا صورت گرفته است ولی تاکنون در کشور ما مطالعه‌ای مبنی بر بررسی نتایج CABG با دو روش on-pump و off-pump منتشر نشده است.
روش کار: 340 بیمار که از 15 فروردین تا پایان بهمن 1380 بطور متوالی در بیمارستان افشار یزد تحت عمل جراحی CABG قرار گرفتند در یک مطالعه توصیفی به روش Outcome Study با پیگیری یکماهه مطالعه شدند. از این تعداد 280 نفر به روش on-pump و 60 نفر به روش off-pump ، به انتخاب پزشک جراح عمل شدند.
نتایج: میزان مرگ و میر 2.6% بود و بین بیماران عمل شده به روش off-pump و on-pump در این خصوص تفاوتی وجود نداشت. میزان استفاده از داروهای اینوتروپ ˂ 12 ساعت (P=0.021)، دریافت خون و فرآورده‌های خونی (0≃P) و رویدادهای عمده زودرس (P=0.027) در بیماران عمل شده به روش off-pump کمتر بود.
نتایج CABG با جنس بیمار، سیگار کشیدن بیمار، چاق بودن بیمار و فشار خون بالا رابطه‌ای نداشت. میزان استفاده از اینوتروپ ˂ 12 ساعت (P=0.041) و میزان دریافت خون و فرآورده‌های خونی (0≃P) با افزایش تعداد عروق گرفتار بطور قابل توجهی افزایش داشت. بروز نارسایی حاد کلیه با هیپرتری گلیسریدمی (P=0.048) و هیپرکلسترولمی (P=0.034) ارتباط داشت.
نتیجه گیری: در بیماران منتخب، عمل جراحی CABG به روش off-pump با ایمنی قابل اجرا است و نسبت به CABG به روش on-pump عوارض کمتری در پی دارد. عمل جراحی CABG در بیماران چاق و افراد سیگاری با ایمنی قابل انجام است و نباید به علت چاق بودن بیمار یا سیگاری بودن وی انجام CABG را به تاخیر انداخت از آنجا که بروز نارسایی حاد کلیوی در بیماران دچار هیپرتری گلیسیریدمی و هیپرکلسترولمی بیشتر است، پایش دقیق کراتی نین خون قبل، حین و بعد از عمل و انجام اعمال پیشگیرانه در این دسته از بیماران لازم است.
